I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Master Dance Worx (Red Rock Country Club), its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Master Dance
Worx”).
I hereby agree to release Master Dance Worx and hold Master Dance Worx harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
I also give Master Dance Worx permission to use my child’s name, image, voice and/or video appear in media available to the public, including but not limited to newspapers, magazines, newsletters, performance programs, websites, social media such as Facebook and Instagram or on any form of advertisement for Master Dance Worx or affiliated events.
If I am a minor, my parent and/or legal guardian has also signed this document releasing Master Dance Worx from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity. Also, I understand that my child(ren) will be under the supervision of the staff during dance class only. I agree to pick up my child(ren) promptly after class time. If excessive tardy pickups become habitual, further discussion with family members may be needed.
The participant has my permission to participate in Master Dance Worx Events. I warrant the below information is complete and correct. I further release Master Dance Worx of all liabilities associated with my child’s attendance at Master Dance Worx.
I understand that participation in dance classes, rehearsals, performances, and events at Master Dance Worx involves physical activity and interaction with others, and as such, there is a risk of injury and exposure to illness, including but not limited to colds, flu, and infectious diseases such as COVID-19.
By signing this waiver, I voluntarily assume all such risks, known and unknown, related to illness or injury. I agree that Master Dance Worx, its staff, instructors, and representatives shall not be held liable for any injury, illness, or exposure that may occur as a result of participation.
I agree to monitor my dancer’s health and refrain from attending classes or events if they are exhibiting symptoms of illness or have tested positive for any contagious condition. I understand that Master Dance Worx may implement health and safety protocols as needed for the well-being of all participants.
Lastly, I understand if my child(ren) do not feel well, we agree to stay home from class(es).
